Please can patients who are attending an appointment with Nurses and Health Care Assistants please make sure they arrive on time. Otherwise this does have a knock on effect to other patients by making the clinics run late. Patients who attend late will be asked to re-book their appointment. Wollaton Park Medical Centre is a 8 partner practice based within the boundaries of the City of Nottingham.

The premises were purpose built in 2007 and we serve approximately 9000 patients. We offer appointments from 7am on Tuesdays and Fridays. These can be booked in advance. Please call reception to enquire. Our phone lines are open daily from 8am to 6.30pm Monday - Friday, excluding bank holidays.

New Patients
Report
We encourage all members of a household to register with us. You will need to complete a questionnaire which can either be collected from reception, or downloaded here. One other form (GMS1) will also need to be collected from the surgery. Anyone can register without needing to provide proof of ID, address or immigration status.
Appointments

We have tried to design an appointments system that ensures unwell patients are seen quickly and patients with long term conditions are able to plan their visits to the doctor in advance. Appointments are always in short supply. This is a result of shrinking funding together with big increases in the average number of visits each patient makes to their GP each year.
Repeat Prescriptions

You or your carer will now need to order your repeat medicines directly from the practice. The pharmacy can still collect the prescription from the practice and deliver to your home if needed. When you next order your repeat medicines, please do take time to order only the medicines you need for the next month and use up any old stock before you order any further items.
General Information
Report
We are a Training Practice. This involves the training of doctors who wish to become GPs. These doctors are fully qualified and are undertaking further training to specialise in General Practice. They are called GP Registrars and work under supervision. Registrars are attached to the practice for 4 - 18 months.
Out of Hours Care
Report
Provides access to treatment and advice for minor illnesses and ailments directly from your pharmacy rather than needing to see a doctor for a prescription. Call 111 if you need medical help fast but it's not a 999 emergency. They will advise you of the best place for you to attend and can also provide health information.
