To make things easier for you to register, download a New Patient Registration Form and New Patient Questionnaire. Complete these at your leisure, print them off and bring them into the practice. It is practice policy to ask for the completed forms to be accompanied by photographic ID and proof of your address.

Cervical smears help prevent cancer of the neck of the womb. Cervical cancer is the most common cancer in women aged 25 to 35. A smear is the best way to protect yourself from the disease. Your smear test can pick up changes to your cells which, if left untreated, could turn into cancer. All women in Scotland aged 25 to 49 are invited for a smear test every 3 years, while women aged 50 to 64 are invited every 5 years.
