
The Woodgate Valley Health Centre has its origins in a modest health centre which was opened in 1972. This stood where our current car park is, and was demolished to make way for our new practice building. It offers us more consulting rooms, more clinical areas, better accommodation for our health visitors and midwives, more administration space and enhanced facilities for teaching.
Since the introduction of the new General Medical Services contract for practices, the practice provides core services as well as a range of direct and national enhanced services for patients. Information about you and the care you receive is shared, in a secure system, by healthcare staff to support your treatment and care. It is important that the NHS can use this information to plan and improve services for all patients. The NHS would like to link information from all the different places where you receive care, such as your GP, hospital and community services, to help provide a full picture.
Change of Personal Details

Please let us know as soon as possible if you change your address, telephone number or name. It is important that we have up to date details in case we need to contact you. Please tell us what changes you have made to your personal details. For a change of name via marriage or deed poll, please provide us with a copy of the appropriate document.
PPG Patient Participation Group

We have a patient representation group so that you can have your say. We ask the members of this representative group some questions from time to time, such as what you think about our opening times or the quality of the care or service you received. We will contact you via email and keep our surveys succinct so it shouldn't take too much of your time.

I understand that this may mean important information will not be available to those treating me when making decisions about my treatment in potentially urgent and life-threatening situations. I understand that by opting out of Your Care Connected I will also opt out of any other local sharing initiatives by default.
