To register at Park Medical Centre please check if you are within the Practice Boundary by entering your postcode in the Postcode Checker to the right. Alternatively, you can print off your own forms from the section below and return by post to Park Medical Centre. If several members of your family want to register, you will need one set of forms for each family member.

Increasingly, patient medical data is shared e.g. between GP surgeries and District Nursing, in order to give clinicians access to the most up to date information when attending patients. The systems we operate require that any sharing of medical information is consented to by patients beforehand.
